ABSTRACT

Pablo Luis Mirizzi was a recognized Argentine surgeon that introduced the opertive cholangiography in 1931 as method of exploration of the common bile duct. This exploratory procedure extended throughout the world. This article remembers the life and work of this notable surgeon and noted teacher.

ABSTRACT

Robotic colorectal surgery has gradually been performed more with the help of the technological advantages of the da Vinci(R) system. Advanced technological advantages of the da Vinci(R) system compared with standard laparoscopic colorectal surgery have been reported. These are a stable camera platform, three-dimensional imaging, excellent ergonomics, tremor elimination, ambidextrous capability, motion scaling, and instruments with multiple degrees of freedom. However, despite these technological advantages, most studies did not report the clinical advantages of robotic colorectal surgery compared to standard laparoscopic colorectal surgery. Only one study recently implies the real benefits of robotic rectal cancer surgery. The purpose of this review article is to outline the early concerns of robotic colorectal surgery using the da Vinci(R) system, to present early clinical outcomes from the most current series, and to discuss not only the safety and the feasibility but also the real benefits of robotic colorectal surgery. Moreover, this article will comment on the possible future clinical advantages and limitations of the da Vinci(R) system in robotic colorectal surgery.

ABSTRACT

La clínica de las afecciones biliares y su cirugía tienen poco más de un siglo. A fines del posado y a principios del actual, se establecieron leyes y describieron síndromes clínicos que tienen vigencia en la actualidad. Algumas técnicas modernas, como la ecografía, confirmaron el valor de la ley de Courvoisier-Terrier. La primer descripción sintomática fue hecha por dos clínicos de Lyon, Bard y Pic, basada en la confirmación autópsica de la clínica. En este trabajo se analizan dichas normas y las excepciones de la ley de Courvoisier-Terrier. También las investigaciones de Pablo Mirizzi, el cirujano argentino que introdujo la colangiografia operatoria. Con esta técnica pudo estudiar mejor los pacientes, transformando a la cirugía biliar en un procedimiento con bases científicas, merced a la documentación iconográfica. El esfínter descrito por este autor resultó ser un artefacto radiológico debido al uso de contrastes oleosos. Finalmente discutem los síndromes de hidrohepatosis derecha e izquierda, descriptos por Catteli y Braasch en la década de 1960. En 1971 señalamos nuestra discrepancia sobre la ictericia en la hidrohepatosis izquierda.
